Healthy You: Chronic Disease Self Management

Lake County Free Clinic invites ANY resident of Lake, Geauga, and Ashtabula County to take advantage of this 6 week course, that is planned around what YOU want and need.

The program is held THURSDAY evenings September 6, 13, 20, and 27, and October 4 and 11, 2012 from 5:30-8p

If you have a CHRONIC ILLNESS or care for someone with a CHRONIC ILLNESS this is a program for you.

MANAGE YOUR OWN PROBLEMS BETTER: Different chronic problems are addressed to help individuals manage their own or a loved ones chronic disease.

Topics include communicating with health care providers and managing medication.

Classes are geared to individual needs

If you have conditions such as diabetes, arthritis, high blood pressure, heart disease, chronic pain, anxiety, migraines and more, this program can help you take charge of your life.

Light snacks. Give-a-ways at every meeting, including gas cards. 4-week FREE Lake County YMCA membership with completion of the program. See attached flyer for more information and other Lake County Free Clinic Wellness Programs.
